Respawn whitelist application

After asking on discord whether the respawn whitelist is time based or not, I've been told to make an application on the forum, I think this is the most appropriate subforum for this.
I haven't found any example for this kind of application so I'm not exactly sure what I should put here, therefore I'll make it short.

I've played here for a bit over a month now, without getting any notes or bans and I'm planning to stay.
